Artery Occlusion
Definition
Disruption of blood flow through an artery causing
ischemia and possibly permanent monocular loss of
vision. A CRAO is an ocular EMERGENCY.
Immediate Treatment
Lower the IOP - AC Paracentesis,
Globe Massage
Iopidine
Carbogen (95% oxygen, 5% carbon dioxide)
If resolved within the first 97 minutes, most vision can be restored.
After 4 hours, vision loss is likely permanent.
Causes
Emboli
Giant Cell Arteritis
Hyperviscosity/Hypercoagulability
Birth Control
Primary Workup
ESR/CRP (GCA)
Lipid Panel (Cholestrol)
EKG (Calcium clots coming from heart valves)
Carotid Doppler (check cartotid artery stenosis)
Secondary Workup
Hyperviscosity Syndromes
Hypercoagulability Syndromes
